TO RECEIVE YOUR CERTIFICATE OF COMPLETION The following MUST be completed and returned to the school in order to receive your certificate of completion. 1.  Download and print the online Instruction Form.  You will need to sign it and return it with your documentation.  Please be sure to include all forms requested.  You WILL NOT be registered if we do not receive all the necessary forms.   Click here to print the Instruction Form. 2.  A completed Enrollment FormClick here to print the CE Online Enrollment Form. 3.  You must send a Student Affidavit to Broadway School of Real Estate.  The Student Affidavit is a signed affidavit from you certifying that you have personally completed each assigned lesson of instruction.  This form is required to be notarized.  Click here to print the Student Affidavit Form here. 4.  A Course Evaluation Form must be completed and returned to the school.  Click here to print the Evaluation Form. 5.  A photocopy of your driver’s license or other photo ID with signature must be submitted to the school as required by the State Board of Private Career Education. 6.  You should also review the BSRE policies and review the 10 Tips for Distance Learning. Final Exams require a passing grade of 70% or higher for course credit. To enroll, you may bring your documentation to the school in person or mail to Broadway School of Real Estate, 1009 W. Grand Avenue, Hot Springs, AR 71913, you may fax to 501.623.8632 or email to kent@keahey.com.  Once you have completed the course, you may return your course materials to BSRE or mail to the address above.  After we verify that you have completed all of the course modules and returned the required documentation, we will issue a certificate of completion.  Your certificate will be mailed via U.S. Mail.
